Peripheral blood in EDTA with a minimum volume of 1 ml is the only acceptable specimen. For optimal results, the sample should arrive within 24 hours of collection at room temperature. |

Complete the Incyte Diagnostics requisition with the patient's demographics and relevant clinical history. Label the specimen containers with patient's full name, second unique identifier, and specimen site/source. Please provide a copy of the patient's complete blood count (CBC) report. It is recommended to retest patients within 6 months when small clones are detected. |
